Our partnership with AP-HP Sorbonne Université (Sorbonne University Hospital Group of the Paris public hospital system)

In Synapse, we have found a dynamic, competent, and reliable team with which we have been able to develop a digitized, user-oriented medication reconciliation strategy in record time. When doing medication reconciliation, the whole process of entering and interpreting the patient’s treatments often takes a long time. This is usually done on paper or in Excel spreadsheets, which may also lead to errors.

Our contacts at Synapse had a great deal of knowledge on the subject and were good listeners, which allowed us to develop an incredibly simple user-and-patient-oriented solution. The tablet-based solution is highly flexible, the workflow between different stakeholders is intuitive, and it taps into the accredited French drug database Thériaque. All of this has allowed us to change the way we think about reconciliation: it is reassuring, and makes our job as conciliators so much easier.

On the heels of this experience, we hope to continue working together on these kinds of subjects and others in the future.”
